Dr. Melinda Fierros, MD
Dr. Fierros is a board-certified Child, Adolescent, and Adult Psychiatrist. She is passionate about offering thorough psychiatric services to children, adolescents, and adults, including medication management, therapy, and repetitive Transcranial Magnetic Stimulation (rTMS). She distinguishes her practice in that she does not limit her patient care to a strict focus on medications, but rather seeks to understand her patients by thoroughly assessing their intellectual, spiritual, emotional, and physical needs.
She views each individual as a whole person and carefully considers a wide array of evidence-based treatment options, from both eastern and western medicine. She has additional training in Cognitive Behavioral Therapy, Cognitive Processing Therapy, as well as Psychodynamic Psychotherapy. Dr. Fierros completed her undergraduate degree in Biology at Texas A&M University in College Station, Texas, graduating with University and Foundation Honors. She then attended medical school at the University of Texas Medical Branch (UTMB) in Galveston, Texas and completed her Psychiatric Residency and Fellowship training at Wright State University in Dayton, Ohio, in a combined military/civilian program.
Dr. Fierros served in the Air Force for 11 years, providing psychiatric services to her brothers and sisters in uniform and their families. She earned multiple awards throughout her education, training, and service, including the Edward Randall Medal for Academic Excellence, the Wright State University Academy of Medicine Outstanding Resident Award, the Distinguished Laughlin Fellow Merit Award, and the Meritorious Service Award. She was the Medical Director for the Military Family Clinic at Endeavors and the Clinic Director for Pathfinders Counseling and Consulting.
She is currently the President/CEO and Medical Director of her private practice, Total Insight Psychiatric Services, where she provides psychiatric services to children, adolescents, and adults in the community. She has served as the Bexar County Psychiatric Society’s Counselor at Large, Vice President, President-Elect, President, and is currently the Counselor At Large/Immediate Past President.
Dr. Fierros loves reading, baking, cooking, dancing, hiking, eating, laughing, spending time with her loved ones, and being outdoors. Dr. Fierros is currently accepting new rTMS patients. Current forms of payment accepted are cash, check, and credit card. Dr. Fierros is empaneled with most major insurance PPOs, including AETNA, BCBS, HUMANA, TRICARE, Medicare, and United. Financing is available through Advanced Care Credit.
Dee Swart, PMHNP-BC, DNP, APRN
Dr. Dee serves as an additional prescriber for the TIPS clinic. She is a Combat Medic/Paramedic and Army Veteran. In 2004, she completed her bachelor’s degree and RN license. She worked in the intensive care unit (ICU) for 15 years. During her career, she recognized the vast disparity of mental health care. In 2019. She achieved her master’s degree with a license as a Psychiatric Mental Health Nurse Practitioner. In 2023, She completed her Doctorate in Nursing Practice.
Dr. Dee is native to San Antonio, has three children, an avid sports fan, active in the gym, a movie buff, and a mediocre karaoke singer with a decent stage presence.
Laura Luna MSN, APRN, PMHNP-BC
Laura is a board-certified Psychiatric Mental Health Nurse Practitioner with a strong foundation in trauma-informed care. Prior to advanced practice, she spent several years as a Registered Nurse at a Level 1 trauma center in Austin, Texas, where she worked extensively with diverse, often underserved populations. Her clinical background includes managing complex medical and psychiatric conditions, with a focus on bridging the gap between physical, emotional, spiritual, and mental health. She remains dedicated to delivering compassionate, holistic psychiatric care, grounded in evidence-based practice.
In her free time, she enjoys spending time outdoors, particularly enjoying local trails and coffee shops in the Austin area. She also enjoys spending time with her family, friends, and her two beloved dogs. You may also see her thrifting, crafting, and reading books.
Carolina Echeverria RN, BSN
Caro has had a lifelong passion for helping others. As a registered nurse, she has been able to combine this passion with her desire to make a difference. Caro started her nursing career as an LVN in an inpatient psychiatric hospital working as a charge nurse for active-duty service members, as well as the adolescent, pediatric and detox units which is where her passion for behavioral health began. Caro moved on to work at a non-profit behavioral health organization- as a case manager for several years while she gained her bachelor’s degree as a registered nurse and now enjoys getting to know the patients at the practice while providing administrative assistance, case management, administering rTMS treatments as well as Spravato and taking part in the development and health goals of each individual.
When not at work, Caro enjoys hiking/walking with her dog Sandy, traveling as much as possible, spending time with family and friends, attending festivals/concerts, and road trips.
